Who processes your data?

Why do we process your data and what authorizes us to do so?

1) Provision of TONSIL CLINIC services We must process your data during the negotiation and provision of TONSIL CLINIC services in order to implement the content of the contract.
2) Fulfillment of legal obligations We process your data in the fulfillment of our legal obligations, which are regulated by legal regulations in the field of providing health services, tax and accounting laws, etc.
3) Protection of our legitimate interests. We may use your data and data on the use of our services to protect our rights. For example, in the case of resolving compensation for damage that would arise during the provision of TONSIL CLINIC services, etc.

What personal data do we process?

Name, surname, date of birth, birth number, place of residence, contact (tel./e-mail), account number, related health data (i.e. special categories of personal data).

How long will we work with your data?

We will process your data for the duration of the provision of services and then for the period specified by legal regulations (decree on medical documentation, accounting act, etc.), usually for a period of 10 years from the termination of our contractual relationship. We will also retain the data for the possible protection of our rights in the event of disputes (the objective limitation period is 10 years).

To whom can we transfer your data?

We may transfer your data to employees and other processors on the basis of a contract with them, but always only to the extent necessary to fulfill the purposes of the processing and to provide our services. If we transfer data outside the EU, its protection will be ensured according to standard contractual clauses.

Your rights

The legal regulations on personal data protection guarantee you various rights in the area of ​​personal data protection. To the extent that personal data protection regulations guarantee this – in particular GDPR – you can request access to your personal data from us (i.e. information about what specific data of yours we work with and how we work with it), restriction of processing of your data (this means that we will not delete the data yet, but we will not work with it) and their correction and deletion (always if the legal conditions are met). You can also object to processing and you can exercise the right to data portability. If you believe that we are violating the law by processing your personal data, you can file a complaint with the Office for Personal Data Protection.

Objections to processing

You can object to processing based on legitimate interest, which we will subsequently restrict unless we demonstrate compelling and qualified legitimate reasons for the processing.

The collected cookies are processed by another processor, namely the Google Analytics service provider, operated by Google Inc., with its registered office at 1600 Amphitheatre Parkway, Mountain View, CA 94043, USA.

These terms and conditions govern the relationship between the health care provider and the patient, or person acting on behalf of the patient (hereinafter referred to as the "client").

Health services not covered by public health insurance

The client acknowledges that the services provided within TONSIL CLINIC are not part of standard healthcare and are provided under the regime of § 15, paragraph 16 of the Public Health Insurance Act, i.e. at the client's request* and are therefore not covered by public health insurance. The client pays for the provided health services from their own funds, i.e. as a self-payer.

Ordering

Ordering is possible via the web form available on the provider's website listed above. The order becomes valid upon payment by the client.

Payment

The client acknowledges that payment for the services that will be provided to the client is always required in advance. For this purpose, the client will be redirected to the payment gateway after completing the web form. The client acknowledges that making a payment is considered a request in the sense of the above (*). In the event that the client does not pay for the services according to the information provided by the provider, the visit date with the provider will be canceled. The price of the services is available on the provider's website listed above.

Change of date

The client is entitled to agree on a maximum of one change of the visit date, but no later than the date and time of the scheduled visit date, without any penalty. In the event that the client does not show up for the scheduled visit date, or in the event that he/she requests a repeated change of the visit date, or requests a change of the visit date after the originally scheduled date, the payment to the provider is forfeited. The client may request a new date within 6 months of the date of the originally scheduled date.

Cancellation of the date

The client acknowledges that by making the payment, a contract is concluded with the provider, who will bindingly reserve the visit date for the client. After concluding the contract, it is not possible to cancel the visit date, only a change of the date is possible as stated above.

Out-of-court dispute resolution

In the event that a consumer dispute arises between the provider and the client that cannot be resolved by mutual agreement, the client may submit a proposal for out-of-court resolution of such a dispute to the designated entity for out-of-court resolution of consumer disputes, which is the Czech Trade Inspection, Central Inspectorate - ADR Department, with its registered office at Gorazdova 1969/24, 120 00 Prague 2, e-mail: adr@coi.cz, website: www.coi.cz.
